Hepatic artery aneurysm (HAA) is rare, the incidence of which is more common in men, between the fifth and sixth decades of life. The majority are extrahepatic, and only 5% are exhibited in the left hepatic artery. It has an incidence of rupture of around 80%, which is the first clinical manifestation in up to 80% of patients. The aim of this study is to discuss the clinical and radiological findings of a patient with hepatic artery aneurysm, diagnosed incidentally, who has undergone effective endovascular treatment and to guide a medical community related to the importance of its diagnosis and proper management, once that the natural history of HAA does not provide the ideal conditions for its early detection, but it must be remembered, as its evolution can become catastrophic.
