An analytical study of enteric ileal perforation- a case report

Back ground: Heal perforation still ranks among the commonest emergencies encountered in the tropics. Typhoid is by far the commonest cause of ileal perforation in India, in spite of the apparent improvements in environmental anitation, showing the lacuna here. The need of the hours in ‘Health Education’ – awareness of the public of the importance of safe drinking water and careful disposal of waste. Also import is the change in Government’s policy to recognize the importance of primary prevention in health care. Aims and Objectives: To study the various factors and epidemiological aspect of ileal perforation ,To analyze the causes, To verify the representation of various aetiologies in ileal perforation. To determine the prognostic factors in typhoid ileal perforation. To study the progression and outcome, when following various treatment methods. Materials and Methods: All 50 patients were admitted as an emergency; routine investigations only were done, in view of the critical condition of patients. The diagnosis was mainly based on operative finding of ileal perforation, which, if associated with features of terminal ileitis and without clear-cut evidence of other aetiological is taken as Typhoid ileal perforation . Results: Our hospital, during the two-year study period from May 2007, has treated 4205 cases of surgical emergencies, of which 50 were ileal perforation. Thus the incidence of ileal perforation stands at 1.189% in Govt. Stanley Hospital, reflecting the incidence in surrounding areas of North Chennai.
