High-risk pregnancy implies a context in which the life and health of the mother and/or the fetus/newborn are more likely to be affected than those of the general population. Stressful events in pregnancy may affect the mental health of women in this scenario. It was sought to explore which are the impacts on the mental health of pregnant women and the repercussions of high-risk on other gestational psychological phenomena. In this narrative review, a search for articles was carried out, published between 2010 and 2020, on the BVS, PePSIC, SciELO, LILACS, EBSCOhost, and Google Scholar. Regarding emotional impacts, high-risk pregnancies were related to mood disorders, mainly depression and anxiety. There are few empirical results that highlight other psychic phenomena in these circumstances, especially those directed at the relationship with the baby and/or involved in the perception of the women themselves. Studies have added to the need for special care in this context. It is suggested to expand the topic, with emphasis on understanding assistance actions that have been being proposed, aiming at better care for pregnant women, babies, and families, and generating positive repercussions in the establishment of relationships.
